When replying to a comment that "mentions" a backlog item, the reply should also automatically/default "mention" that backlog item also
So if you are "following" someone who replys to something, you can easily see what backlog item was being discussed even if you weren't following the original commenter
Currently if someone replys to a comment in a conversation linked to a backlog item, it doesn't automatically "mention" that item, so its hard to track in the main conversations feed.

I agree with this. It can sometimes be difficult to find items that were "mentioned" in a conversation. Easy traceability from conversations back to their "mentioned" items is critical for usability of this feature.
One solution to this might be to have a thread-based view in addition to the current stream-based view. Mentioned items could be associated with each thread so the user wouldn't have to manually search for the first item in a conversation.
Another source of confusion has to do with what happens when you click on a “mentioned” item in a conversation. Everywhere else in VersionOne, this brings up a dialog of the item. In conversations, this brings up another screen with all the conversations attached to the item, and the user then has to click the "Details" link to get to the item. This type of modal behavior should be avoided, in my opinion. Instead of clicking on the mentioned item to get to the list of conversations, there should be a separate link for this purpose.
I'd like to have the flexibility to either "reply" which is already there and does not auto-mention, or to "reply all" which would auto-mention everyone and every backlog item that was included in the previous comment... that way the user can determine who needs to stay in the loop.

Yes, this is very important. Without the automatic mention of the original item, you click to a page that shows you a list of unrelated conversations.
I concur - "Reply" and "Reply All" would be very helpful to keep the thread tracked within the Version 1 Conversation. Currently, users reply to the conversation email in Outlook and address it to the necessary parties because it's faster in Outlook compared to adding them in the V1 'mention' field.

When is this happening?
This has been addressed with the redesigned threaded conversations in asset detail views. The new design shows threaded conversations and embeds interactions inline. A conversation thread will be displayed on backlog item whenever any expression within that thread mentions the item. This means that subsequent replies need not mention the item again for that reply to show up on the backlog item's detail view, it will just be pulled in as being part of the thread.
